You do this be doing the Do Re Mi Fa So La Ti Do thing.
Practice really high notes & really low notes.
You probably won't notice for a while but it will eventually widen your vocal range.
That's why it's important to do everyday.

well...first the identification. practice near a piano or some other well-tuned instrument and check yourself every few measures of whatever piece you are practicing. you may start to notice some patterns that way, such as going flat in high or low vocal extremes (relative to your own voice), or in between certain notes in your range. certain vowel sounds can tend to drag one's voice flat, also.
once you have identified you own personal tendencies, try to fix them yourself. start with good posture, deep "diaphragm" breathing, and good postion of tongue, throat and pharynx. if you are not familiar with that stuff, find a teacher...if you live near a college, you could probably find staff or even grad students there who could help you.
in the end, a teacher would be best. someone who can listen to you every couple of weeks and tell you what you need to do to be the most in-tune singer you can be.

Flatness can come from too much pressure on your vocal chords. Make sure that when you sing you're not pushing or hurting yourself and that your body is well-aligned. It's very important not to stick your head forward when you sing, as this can put strain on your vocal chords. If this doesn't fix it, it could be your ear and trying to think of the notes as a half-step higher could help.

You should practice with either an in tune piano or a electric tuner. Use more air and even doing something physical such as simply raising your eyebrows could help you visualize a sharper note/

Here's one that you'll probably never hear....
Your posture may make your voice flat. Make sure you are standing or sitting up straight and sing straight from the diaphram.

Are you making sure to sing from your diaphragm? Try doing lots of exorcises, making sure you are singing correctly.
