Piggyback infusions are solutions or dilutions given through a secondary line. The primary line would be the infusion directly connected to a tubing going to the the venous access. The piggyback is going through another set which tubing will be connected to the injection port of the main line. When piggyback infusion is given, the piggyback unit is hung on an IV pole at least 6 inches higher than the primary container.

You have a main IV infusion going--such as something as simple as normal saline and you need to run a second, small medication in, such a gentamycin; you get the small antibiotic bag and attach its tubing into the main IV portal at a Y and run together--essentially using the NS as a carrier for the gent. Some times you have fluid limits a patient can get so piggybacking is a way to minimize fluid loads. Some times the IV is caustic and a piggyback will help dilute it. Most important when piggybacking is that both solutions are compatible and will not cause a precipitate. (This is one reason why you can't piggyback onto blood or TPN).

A primary infusion would be one thats running continuously such as sodium cloride and and piggy back or secordary bag would be hung at a specific time (such as a antibiotic) for a short duration, usually a hour. The primary would kick in after the secordary is complete

a piggy back infusion means the medicine is being given secondary to an existing infusion. some medicines need to be diluted with water for example. So if you have an iv infusion of water already running, i would start the second medication to run in conjunction with your existing med. or piggy back it.
