You need your birth certificate. They take it and send it to the passport agency. Don't worry, you will get it back. You also need your social security and ID. Oh and a picture to go with the ID.

Birth certificate, social security card, and photo ID (ie Driver's License) and if you are married, your marriage certificate to indicate name change.

Birth Certificate should suffice and your Social Security Card wouldn't hurt either.

What category is this in again? I thought it was me. But i think you only need one form of ID and a birth cert. I don't know but this link will surely help you, check it out.

here in California, you need drivers license, birth certificate, two pictures of head shot...2"x2"..file form and about $100 buck fee ....one can file at the local post office...I just dropped mine off today...paid an extra $40 to expedite...should have it within 30 days.

depends on the country and the requirements they impose --- check online -- there will be a passport section in the governement website and they will tell you the procedure you need to follow and the forms, photos etc that you need to provide and if you need an appointment or not --- have fun
