It's supposed to be about the bubonic plague, but it's an urban legend, false.

It is symbolic for the black plague:
ring around a rosy (people with the plague got red 'rosy' rashes on their bodies, in a circular shape)
a pocket full of posies (posies were thought to combat the plague so people carried them around in their pocket)
ashes, ashes (they burned dead victims so the plague would not spread)
we all fall down
