"M" is a code letter and name for the head of the British Secret Intelligence Service, or MI6 (or MI7). The title "M" is believed to have been derived by creator Ian Fleming from the first real-life head of His Majesty's Secret Service, Mansfield Smith-Cumming, who used his initial "C" to indicate when he had seen a paper and so became referred to as "C", a practice that carried on with his successors. The "M" may also be an abbreviation for Minister, referring to his title as head of MI6
The actress currently playing "M" is Dame Judi Dench.

I'm not sure Judi Dench's "M" has ever had her real name revealed. It could just be title that started earlier in the book or film series that has been continued for consistency. "M" as played by Bernard Lee is named Miles. It's brought up in "The Spy Who Loved Me." General Gogol and M are together and each addresses the other by his first name.

EDIT: Moneypenny is actually the personal secretary of M. In the novels, M is revealed to be Admiral Sir Miles Messervy. But now that M is a woman in the recent movies, there's got to be another name. I'll keep looking...
EDIT: A Bond novelist said it could be Barbara Mawdsley. It is said to be verified by IMDB, but I could find nothing.

It's never revealed, though Bond does state that "M" stands for her name; he begins to say it (starting with "M") before she cuts him off.
