If I am writing a book based on interviews, where can I get a waiver to have those being interviewed, sign with knowledge that I am writing this book to make money, however, they have no rights to the revenue?

Create one. It isn't all that hard. And have them sign it before you interview (it will save you a lot of head aches later).
Also, be sure to include the clause about them not being compensated via royalities (because it can be assumed you are writing this book for money). Be careful on who you choose for your subjects though. Some publishing houses will not touch a book if the subjects are protected individuals, such as children or prisioners, unless certiain certificates are held and certian processes are followed.
Depending on what you are writing about you'll need to be NIH Certified. http://cme.cancer.gov/clinicaltrials/learning/humanparticipant-protections.asp
This link will take you through the test to get your certificate.

Good Luck!

Literary agents and photographers will have these, or you can also find examples on the Internet. Finally, you can make your own release form but it has to be clear.
