The tourist card I entered Panama with was good for 90 days, but they just changed the regulation to 30 days. You can renew it by going to the Migration office in Panama City. One of my friends went over her visa by a few days and she only had to pay ~$10 fine, so it's not a big deal if you go over, just an extra little hassle.

Go to the Migracion offices on Avenida Cuba at Calle 29 Este (there are also offices in David, Chitre, Changuinola and Santiago). You'll need: your passport, $10, a photocopy of your passport page containing your personal information and the page showing the stamp of your most recent entry into Panamá, return ticket and a letter requesting the extension ( which you can write on the spot.) In Panama City, when you arrive at Immigration, pull a number and when attended, ask for a "prorroga de turista" which is the form you have to fill out.

Have you left yet? If not, talk to a travel agent. If you are already there, you'll need to ask the authorites (I think it'd be the immigration office....but I'm not certain). I was in Panama years ago for longer than the 4 weeks and I went to an office for an extension. But since it's been so long, I forget the name of it.
