She will have to call her service provider's customer service. They are the only people that can give her the PUK code. They will verify that she is the account holder, and ask her for some information from the sim card and phone. After everything is verified they will give her the PUK code. Tell her to make sure she stays on the line with them until she gets it unlocked.

FIRSTLY:
DO NOT do anything further with the phone otherwise it will be "rejected" i.e. SIM will become useless, even PUK # will not help and you will have to buy a new SIM.
SECONDLY:
Go straight to the Service Provider. They can bring it back to normal using PUK # they have. It is a unique # of a SIM.

she needs to call her service provider and get the pin unlocking key or puk if the incorrect code is entered too many times her sim will be permanently locked and will need new sim. for about 25.00 bucks

Call the service provider or manufacturer and ask for the unlock code. You can try to unlock it yourself, just research unlocking (yourphonesmodel) and I'm sure you will find some software or guides on the internet. Good Luck!

I did the same thing to my sisters phone, what you do is look in the manuel just to check to see if there is a code if not go to the place where she bought the phone in my sisters case at cell one we went there told the guy what happened and he will have the code and everything and then the phone will be unlocked trust me go to the place where she bought the phone!

have her call her service provider, don't mess around with it or it will lock (forever). I had the same problem almost had a very expensive paper weight...it should take about a 5 min call plus wait time.
