Our thoughts, and normally subsequent actions, if are considered as expressions of some innate self to our mind that is in constant interaction with the world outside then we can see that there are very distinctly two entities in existence for every rational human being. If the world is the place that our thoughts and actions are purposed for then the question is, what is the essence that conscious mind, subconscious mind, ego, intellectual and physical abilities are in line to server?
We can see that our conscious mind is like a front room of our house, if our personality is the façade of the building. Then we can find out that our subconscious mind is like the inner quarters of our house where we keep things gathered up over the days, months and years of our life as they pass. There are for example things in the attack or in the basement that we no longer remember are there, but can be recalled whenever a need arises. Then there is our high mind, or supper mind, the contents of which are no longer material in nature, and need no physical storage space.
This is the stuff that governs our life, the values we choose, the beliefs we have and relations we entertain in our life. This is where our thoughts and actions find their ultimate reason, the essence that some people also name as soul. The essential purpose of soul is to reach excellence in being, to constantly seek and find the absolute in the nature of all things, contentment and peace of mind is in its commitment to the purpose of soul.

As a poem, you have some unusual grammar usage -- It should be consistent throughout. For the flow of a poem, capitalizing each line is passe; it was done in Jolly Olde England because the peasants couldn't read or read well; the only way they knew it was a poem is by the capitalization of the first letter of each line. If you go for the flow and don't capitalize each line, then be sure to use periods to indicate the end of one thought. It is a great idea, and may come to pass -- but I don't want to be here if it does because I prefer individuality. Last stanza throws me... and I'm not sure where to go from there. To me the whispers never end In judgement for my errors But When we stance and take our place <-- when capitalized We shine through our endeavours <-- also stance in line above -- stand? (another good place for a break, not needed, but I would) Lightening strikes once everyday But I plan to do what's right To take the blame, the hurt, the shame <--added comma My will is my advantage <-- should have period Confidence and living hope That others will join the plan To show the way, so that one day We all live out as one <-- I'd put a break here (with a period) It’s not that far The truth we seek Its all begins at home, <-- added a comma Inside the shell Our living hell Our conscious mind, abode To each and other It all takes shape That defines the one we are Open the gate to the secret place The one that hides your heart Let it breathe so it can feel Have empathy for forever <-- have eternal empathy -- it avoids the for for... of the line, yet you do get your alliteration. Let them in to share the act Of love for one another

I bet it's our conscious mind that's responsible for most of our thoughts and actions, and the so called Freudian slip is our real hidden thought of our sub conscious, but sometimes slips out oops, unintentionally, (sure it does, doesn't it ?)

the actual thoughts which allow you to have thoughts, interactions with others and complete activities are directlecty moved by the conscious mind - the basic precepts of how all of those initially come togther are derived by the sub conscious

It does not "control" them. It takes the environment around it, and spits out words and images and emotions into your brain. Its a simple mechanism that we take to seriously. If you want control, then let them be - observe your thoughts and actions, but do not get too attached to them as being "your own" - they are only a simple product of your genetics, circumstances, and social conditioning. It is strange, but once you accept this, you actually end up with much more control because you are able to look beyond your thoughts and actions and realize what is real and what is not.

The conscious mind runs in what is called "Beta" creating brain frequencies between 13 to 30 cycles. Here reason, logic, and choice can be controlled. When you're in the Beta state, you are focused, and have a goal
The next brain frequency is called "Alpha" and runs between 8 to 13 cycles. The subconscious mind runs here and creativity, Imagination, and subliminal messages thrive. In the Alpha you are awake, but not focused, and more in an awake daydream phase. It is in the Alpha phase where you can hypnotize yourself. Marketers are well aware of this, and commercials are designed to put you in a mini-dream like state so they can hypnotize you. This is simply, relaxing you, or elevating you and repeating a message over-and over- and over. This phase is the moret controling than Beta. Since you've already mastered walking, you're in the Alpha mode to walk, see path your on, watch out for steps/tables/rocks ... and possibly using your Beta mode to manage your "To Do list" while walking
The next phase is "Theta" where brain frequency is 4 to 7 cycles. Deep hypnosis occurs here to obtain deeper functions like controlling body pain. Here, the person is in that first phase of sleep, and doesn't retain memory of what's happening around them (like in the Beta & Alpha phases). This controls even more body activity as it handles breathing, muscel control, long-term unused memory. So, while your walking, Theta is controling life sustaining functions, Alpha is controling functions you've already mastered (walk/run/ joke) and Beta is what you're current agenda is.
The last phase of conscious is "Delta" with 0.5 to 4 cycles. Basically, you're comatose at this level.
