if the surge protector has modem jacks simply plug from the wall to the surge protector.
The other jack on the surge protector will go to the modem in your computer ( not network card the bigger jack !!!)
on your modem will be a phone jack
It will have a picture of a phone on it
( there will only be two jacks on the modem for the computer)
plug your phone into that as an extension phone

Unplug the phone line from your modem and plug it into the "in" jack on the surge protector. Get another piece of phone line and run it from the "out" jack of the protector to your modem.

hi, you connect it in between the wall and the computer
phone jack on the wall -----> surge protector ---->>> pc modem
(you will need another phone cable )
if you use DSL, then:
phone jack ----> surge protector --->dsl modem --> pc
same with power
ac power from wall ---> surge protector ---> pc\dsl modem\monitor\speaker adapter\ anything that uses ac power for the pc
