Can't answer every thing but I can say something about the method of spread.
the latest about prion diseases is that they discovered that these prions (which are simply protein molecules) are encoded by DNA that is native to human DNA but it doesn't get active unless you are exposed to the prion itself.

So diseases like Mad cow disease occur in those who carry the gene for it, the question is how this gene is turned on,some say that the exposure to the prion itself induce the gene to make more prions (most popular theory) while others say that other chemicals found in animals do that.

The disease however spread through persistent contact with row or atleast undercoked meat (espcially nervous tissue). proper cooking destroy the prion proteins and thus render the meat relatively safe.

in animals the disease spread through feeding cows a feed that contain meat products from other cows. it is a common practice to grind the useless animal product into animal food to enhance the protein content of it. this I guess is now proibited
