Both are the abbreviations used for the words 'Peace Be Upon Him' and 'Subhnahu Wa Ta'ala' respectively. The former used with the names of Holy Prophets (All of them without any distinction) and the later with Allah.
'Peace Be Upon Him' serves two purposes: one; its a sign of showing respect and honor to the Holy Prophets. 2nd; it is a prayer asking peace and blessings of Allah for them. One person here asked why to say pbuh unless he's in trouble? Well, to pray for someone does'nt necessarily requires him/her to be in trouble. One can pray even without any reason. And praying for peace is the same. Secondly as i mentioned earlier, it is also a form of showing respect and honor towards them.
'Subhnahu Wa Ta'ala' is an Arabic word, 'Subhanahu' meaning The Glorified and 'Ta'ala' is derived from the word 'Muta'aali' (one of the 99 names of Allah) meaning 'The High Exalted'. The reason why it is used everytime Allah is written or spoken, is to testify and accept HIS greatness everytime HIS name is mentioned.

PBUH means "peace be upon him", we say this when talking about any of God's prophets and SWT means glorious and exalted is He (Allah).
We also say "alaihi salaam" (A.S.) which also means peace be upon him when mentioning God's prophets.

Muslims use PBUH meaning Peace Be Upon Him after names of any Prophet in Islam (Adam, Noah, Abraham, Isac, Ishmael, David, Solomon, Moses, Jesus, Muhammad etc). in respect of them. Since they all deserve respect.
SWT is Short of the word in Arabic 'Subhanahu Wa Tala' meaning ' Glorified is He High Above' This is used after name of God Almighty- Allah. Meaning God is high above all those who blaspheme in His name like having a son or a partner. Since in truth He is One and Only.

(pbuh) is the short form of PEACE BE UPON HIM...

It is the English translation of an Arabic phrase "Alayh is-Salaam"; Muslims mention this phrase after taking the name of Prophets and Angels.
