No, they do not always run away. They have, on average, excellent sense of direction and great loyalty.
But a cat MIGHT run away if you let them out. Or it could get hit by a car, or fall victim to a predator (a dog, another cat, a human). It can catch fleas, it can catch diseases, it can eat something poisonous.
If you own a cat, it is your responsibility to keep the cat safe. That is best done by keeping it indoors at all times, unless you take it for a walk on a leash (yes, cats can be leash trained).

I have 5 cats. They love to play outside in the backyard. I have never had a problem of them running off.If a cat is secure in where he lives, it is more likely that he won't go any further than his own backyard. They definetly have a sense of direction. They know where they live and who cares for them. They will return if they happen to stray.Unlike some peoples opinion I think cats are very loyal, they give love to you just as you give love to them.Gloria S

My oldest cat tries to sneak out when the door is open. When he does go out he just rolls around in the grass and comes back in about a half hour or less. My 2 younger cats I try not to let out because I am not sure what they will do or if they would come back.

My cat stays in my yard when I let her out. I think cats have a sense of direction and they earn to stay near their house.

A lot of cats will stay near home, because they know when they are home, its warm, safe, and they can get attention and food there.
If they are male and haven't been neutered, they will wonder.
But pretty much all cats have a sence of safety and direction.
It's called survival.

They have both! Most cats do not run away. They are usually hit by cars or injured by other animals. I suggest indoor cats.

Sometimes my kitten ran out when the door opened but it was easy to catch her because she wasn't used to the environments and got scared and ran back inside. My other cat he always comes back because he was used to where we lived but of you move he wont no where to go and might get lost.

they are really not loyal like dogs, and they like doing things by themselves. however they do have a sense of direction, and they are very good at finding their way home.

My cats practically lives outside (I know, there are leash laws...) & he always stays around the house. Pretty much at any time we call him, he finds his way home.

Don't risk it. Not only can they get hit by a car or injured by another animal, consider how mean people can be.
