A fellowship is done after residency, and it is in a narrower field than the original residency.
Examples:
internal medicine residency - cardiology fellowship
general surgery residency - cardiothoracic surgery fellowship
anesthesia residency - pediatric anesthesia fellowship
Some (crazy) people do more than one residency, and/or more than one fellowship.

Residency is a training program for doctors who would like to specialize in a certain field of Medicine like Pediatrics or Internal Medicine. Doctors who finished residency may undergo fellowship training program as a subspecialty after the residency training program, e.g., Pediatric Pulmonology.

They are the same thing. In Canada when you are going into a specialty of medicine after completing you M.D. it is called a Residency. In the USA it is called a Fellowship.

Residency, is a formal state of being a postgraduate trainee, stepping in the ladder year by year,,,,in short words, you are an slave with no rights, until you graduate as an specialist..(then you can be hired as an Attending Physician, who is a kind of semi.god to you)...
A fellow, is an already graduated specialist, that is being trained into a subspeciality,but in the meantime, he is dedicated full time to that task, and cannot act as attending or independant specialist,,
In few words, a fellow, is an specialist being trained by other specialist in a particular field....
