My lil girl is due in October. i'm in the military and i deploy in September.
Do i have to sign the birth certificate to get my name on it?
Does my name have to be on the Birth Certificate to enroll her in DEERS?
Can I right an affidavit of use a power of attorney to get my name on the certificate?
Do i have to do a DD form 137-4?
Shes going to have my last name and I'm not married to her mother.

Call DEERS, or go to your local DEERS support center. They will either have the answers you need, or know where you need to go.
By the way, why wouldn't the mother put your name on the certificate?

Some states have very strict laws were if she isnt married then the child will have her lastname and then when you return you'll have to go to court to change it. Im a Marine. As frustrating as it sounds its there to protect to, too many woman in the past gave a child a lastname when the man wasnt present because he didnt know or believe he was the father. He would then have the hastle of going to court to fight that he wasnt there for the birth and that his name shouldnt have been on the certificate and hes paying child support for a child that isnt is, until the paternity test is taken, worse woman that have affairs and the child isnt her husbands give birth while pending divorce and the child automatically gets the husbands name because they are married but in some states even after they prove it isnt yours, you dont get your money back. So you can go to Deers but they cant provide your child care until the child is born. The child even though of course under the age of ten will get her own paperwork and ID for the mother to hold and use. Depending on the hospital and state shes in she wont be allowed to put your name on that birth certificate if you arent present or not married so go to Deers and Legal office ask them what procedures you can do for medical coverage and see what type of a power attorney you can leave her so she can then to your admin and get your child ran in the system and to start to get BAH partial and other entitlements for your child ran in the system while you are gone, also when you get entitlements make sure your child gets money if not you wil receive a big time checkage. Also if that doesnt work, while your deployed there will always be an admin office and they can forward it for you have her mail you copies of documents and you can have her added to your Service Record Book as a dependant and get your entitlements that way. Congrats on the baby girl, congrats you arent marrying just because of a pregnancy.

Not sure if it is the same in every state but there is an affidavit that men sign when they aren't married to the mother that acknowledges them as the father and then they are put on the birthcertificate. There is a paper that the father must sign and the fathers SS# has to be given and all your information. There is a paper that the mother should be given that asks all the questions. My now exhusband is in the Marines and he wasn't present for the birth of our second son and I think the hospital said he couldn't be on the birthcertificate without signing some paper. I know they woudn't add the boys to the DEERS without birthcertificates and they said their fathers name had to be on it but I know my friends new husband has her two kids enrolled and the kids aren't his so I would contact the DEERS people and ask them about the birthcertificate and maybe inquire about it at the hospital she will give birth at and see what they say about the birthcertificate. Is she giving birth at a military hospital or civilian hospital?

If you are married to the mother of the child, than your name will go on the birth certificate automatically if the mother has proof that she's married. Have her take her marriage certificate with her to the hospital to prove that she's married, and they will put your name on the certificate, no questions asked.
If you are NOT married to the mother, you will have to wait until you return and file an affidavit of paternity with the Vital Statistics Office in the state where the child is born. The fee usually runs around $20.00, and it only requires a notarized form that the state office can provide for you.

The mother is usually asked the father's last name, if she doesn't want his name on the birth certificate she can say "unknown". But I strongly suggest that his name be put there for child support

This is one time your child may suffer because of your indiscretions!! If it is possible for the two of you to get married before you go over seas and you love the mother to be then that seems like the best answer. If there are hurdles you are not telling about such as one of you are married to someone else, then you have to just trust that things will work out and learn a lesson from this my friend.

The best advice I can give is to contact DEERS. See if you can get an affidavit before you deploy that you can fill out and give to the mother of you child. I want to commend you for being a good father. So many guys in the military do not want to take responsibility of their kids. I speak from personal experience regarding my baby's father. Marry the mother of your child man!! geez if something happened to you don't you want that little girl to be taken care of... why commit to the child if you can't commit to the mother.... then this wouldn't be an issue for you

My husband also in the military thanks for serving our country. He was not at one of our son birth but his name is still on the birth certificate. Were it says father name your name goes there only one parent sign. That's in Ca

Why not get married? You have already commited your life to her by getting her pregnant with your child. Why shouldn't she have the right to have the same name as the baby?
