It's the next stage in a relationship. You go from new girlfriend to old lady. It doesn't mean your old it just means you've been around a while. It's not really an insult or a complement it's just kind of a funny way of saying your significant other.

It could mean that the person is my wife.LOL... Not necessarily that she is old, but a way also perhaps to say that the lady has been "with me " for a long time.
