Contact each bureau by US Mail with return receipt.

Write a letter with the following information.
How you found the incorrect information.
A copy of the incorrect information with the wrong name highlighted.
A copy of a legal document with correct information highlighted.
Request a copy of the fix be sent to you and anyone who has inquired on your credit in the last year.
Request all this be done within 30 days.

When you get the fix, it should come from each of the three bureaus. Make a copy of each documented fix and keep them for your records.

Wait 90 days and make sure the fix stayed in your records. If not, start all over.

If you have any additional questions please feel free to contact me.

Go to the credit reporting company and tell them to correct the information. May be necessary to check all three.

You dispute it -- with the credit reports -- and they will most likely have you send something in with your real name or whatever- but, do it with all 3 credit reports if its wrong on them all - otherwise- do it with the one --..
its free to dispute.
