They can make a difference, but often what you really need is a new pair of shoes or a new brand of shoes as the padding, thus the shock absorption, wear out after long term use. Foot pain that is caused by shoes also tend to be caused by shoes that do not fit well, so get fitted properly. See a doctor if need be.

I have found some mild improvement using the Dr. Scholl's gel inserts and memory foam inserts, but I stress that it is MILD improvement in how good the shoes feel and how long they are wearable during the day. A good shoe and a good fit is the most important thing as most decent brands have very good cushioning designed into both the shoe and the inserts. I just got some Nike Shox, and the little shock absorbers on the heel are no gimmick. They feel great! My dad talked me into them as he uses them for his daily walk. Since he got them, his foot pain and knee pain after long walks have not only been reduced, but eliminated.

A warning: I recently tried the new Dr. Scholl's Tri-Comfort Orthotics, ones that were "clinically proven" to reduce heel, arch, ball of foot pain. They are 3/4 foot length to allow more movement in the toes. I HATE these insoles. They feel okay while walking but no better than the insoles that came with my shoes. The problem is that they seem to CAUSE pain when you are not standing. I wore them for a week and was unimpressed. Then, on a long car trip where I was just sitting in the back seat, my feet started hurting with these insoles. I've never experienced foot pain from sitting! They hurt so bad (on the balls of my feet especially) that I took them off and wore flip flops for the rest of the trip with no more pain.

I work as a vet assistant and I'm constantly on my feet moving. I had pretty regular foot pain, especially in the balls of my feet. I started wearing shoes with the Dr. Scholl's inserts built in, and I haven't had any foot pain since.
