Well mostly its done by laproscopy which are a couple of small incision near the belly button. They go in with a camera and the necessary tools and do it with minimal invasion. Older procedures involved a 3 - 4 inch incision in the vicinity of the appendix. I date a guy who had like a 5 inch scar.

There are different ways of approaching the appendix.

The classic way is through McBurney's incision, which is a diagonal incision in the lower right quadrant of the abdomen.

The newer way is via laparoscopy, which uses several small incisions to place laparoscopic ports.

Sometimes surgeons go into the abdomen not knowing what they'll find (exploratory laparotomy) and that usually entails a midline incision.
