It can mean either "it's (that's) not good/alright" or "isn't that good/alright?"

In the first case, your friend might say, "let's go eat ramen.. ii?" (okay?/is that alright/good?) and you'd say, "Ii janai," meaning, "No, it's not good/alright." It's a bit abrupt to say, though.

In the second case, you might say to your friend, "I'm hungry," so she might say, "Okay, let's go eat ramen. Ii janai?" meaning, "Isn't that good?" You could also take it to mean "isn't that a good idea?" or "how about that?" although it doesn't literally mean that.
