Although it is unusual to get a free upgrade, sometimes it can happen if the flight is overbooked in economy class and there are empty seats in first/business class (usually on international flights at peak summer season.)

In the case of an overbooked flight, the checkin agent will take note of "suitable" passengers to be upgraded, ie. frequent flyers, well dressed polite passengers, no young children etc. Then if everyone shows up to checkin, the suitable passengers will be upgraded at the gate, leaving enough seats for all.

However it is different which each airline, for example if you are flying on a U.S. domestic flight, there is a different system whereby you can pay extra at the gate before boarding, this is a promotional upgrade. These can be quite expensive though and arent always worth the extra money, example from New York to Florida we were offered an upgrade to business class for an extra $100 per person. It was only a 2 hour flight and the seats were only slightly larger, but the service and food were better.

All airlines have a strict policy regarding upgrades, and simply its no No NO, but rules are created to be broken and getting in first class might not make you that lucky!
If something goes wrong during your travel and the airline is to be blamed can get you and upgrade, e.g. a guy stepped on a nail or a screw that was coming out of the floor of the aircraft, although it was very minor wound he still got upgraded on the return part of his travel, another way if your reserved seat on economy was sold and you have no other seat but first, the flight was late and you lost your connection the next flight have only first class seats.
As for paying this is very rarely to happen, airlines make sure to make big $$$ out this they wont give it for free.

If the airline oversold their economy section they and have not filled the business or first class section they will bump people up so they do not have to bump people off the plane. First they will look for the person with the most status (people who have frequent flying points and fly the most) then they will look at people who have paid a full fair insted of people who paid the reduced/with restrictions fair. After that i doubt there will still be space

because some people have to wait for hours and the airline bumps them up so they will keep flying with them
its a complimentary thing like getting a hotel night free

usually, airlines would looked for premier members first. if there are no more, than they would look for well dressed, polite, one who does not have too many special request such as special meals, seat, etc, etc and who does not make a nuisance of themself during check-in would usually be considered for a upgrade if the flight is overbooked. if you are rude and demanding, they would quickly take your name off the considered list. so be nice to the check-in agent, and hope for the best.

Yes. Happend to me.

Here is another little trick.
When you get your boaring pass, you talk to the agent. Ask them are they any open first class seats. If they say yes. Then this is what I do. I will give you $20 if you change my seat. 80 percent of the time they will take the cash and give you the ticket. They are not the highest paid people in the industry and the $20 helps them out.
It works.

When I check in , I use one of the automated kiosks. It offered me an upgrade to 1st class for only 20 dollars one time and I took it. Best thing I ever did. It doesn't happen every time though. I always cross my fingers when I check in now. Can't wait for it to happen again!
