I know everyone has to have one, but what about babies?

yes if you are traveing out of country a baby can not travel on a mothers passport i had that problem with my baby

Yes a child needs a passport at any age now. Babies are part of "everyone".

Depending on the country you are traveling to, you may need a documented permission slip from the father if he is not traveling with you.

Due to the theft of infants as well as nasty custody battles, I suspect that you will have to have a passport as well as well-documented permission from the baby's father/mother, whichever you ain't.
