I normally use supershuttle from my apt in the city, but I'm staying with my girlfriend in Astoria and apparently they won't service that area.

If you want to leave home at 4AM you have to take a cab...public transportation doesn't start running into NY City airports until 6AM. You can however, reduce your cost by taking a train or bus that gets you closer to the airport and take a cab from there.

Get the R train to Roosevelt and Jackson Avenues and transfer to the E train to Sutphin Boulevard in Jamaica. On the street level you'll be able to get a cab (the drivers hang out there because it's a major hub).

If you can change your flight time, the AirTram leaves from Sutphin Boulevard starting at 6Am, costs 5 bucks and takes you to all the terminals in JFK.

I live in Astoria & find that a cab is quickest. You could take the Airtrain to the subway, but unless ur gf lives along the R line, you'll have to transfer a bunch of times to get the the N,W lines. Besides, who wants to take the subways with luggage at 4AM? It usually costs about $40 in a yellow cab. NEVER call a car service, they charge way too much. And the yellow cab stands are all over JFK. At 4AM, you'll have no problem getting one.

Call a car service! Most are open 24 hours.

A cab will be very quick at that time or a car service.
