Anemia can come from many things. If he has any internal blood loss it will cause anemia. If his bone marrow is not producing well, it will cause anemia. If he is loosing blood in his urine or intestines, anemia will result. Even parasites or worms cause anemia. What was his diagnosis from the vet exactly? Is he in kidney or heart failure? Does he have a cancer somewhere? That will certainly cause severe anemia and organ failure.
Your feeding schedule didn't cause this. You have been a good owner, getting him health care when he needs it. He is just ill in some way. Don't feel guilty.

It may be Immune Mediated Hemolytic Anemia, where his own body doesn't produce enough red blood cells or when the red blood cells don't carry enough hemoglobin. It could also be because of his liver disease. Liver disease is another factor with Anemia. It could be because of a parasitic invasion. Most likely it's the liver and that is why he was put on a special diet. So that he could digest everything with the correct enzymes that the liver produces. If you keep him on his special diet and take him to his regular vet visits he should be just fine. Good luck.

You're not going to be able to get that answer on here I'm afraid, you'll have to direct the question at the vet who diagnosed the condition.

Anaemia can have a number of causes, but since your dog has these organ problems I would guess that the anaemia is secondary to them. Liver problems commonly result in anaemia and/or blood clotting problems. Organ problems and anaemia, especially in young animals, is commonly hereditary. They can also be caused by viruses. As I say, only your vet can give the diagnosis for your pet.

Anemia is mostly likely caused by his liver problems. But it's not unusual for animals to be anemic because of flea or tick infestation, as well.

over population of fleas and ticks can cause it, look at the dogs gums if they r white (instead of pink or red) then he is low on iron. Keep in touch w/ your vet though. best wishes
