Would a 4 millimeter hole in a tooth have started from the inside of the tooth or the outside? Curious, because some say, for instance, that lightning comes from the earth to the sky!?

That's actually a really good question.
There are two main layers to a tooth in which you can get cavities. Both layers serve to protect the inner living tooth tissue called the pulp, where blood vessels and nerves are. If the inner layer of dentin is compromised for any number of reasons, maily genitics, they can start on the inside, and work their way out. Cavities always have an external beginning, although the internal dental layers tend to decalcify and soften first, it usually seems very intact until in further phases, when by mechanical effects, the external layer breaks and the cavity appears.

Cavities come from the outside of your tooth because of a build up of plac and other residue on your teeth that start to eat away at your tooth. Lightning is different because it has to do with different charges between the earth and sky.

Dental cavities comes from the bacteria S. mutans. The bacteria live only on the teeth and cannot colonize the mouth in absence of teeth. They produce glucans, a polysaccharide composed of repeating subunits of glucose, which is essential for the production of dental cavities on smooth tooth surfaces. When sugar enters the mouth it drops the cariogenic plaque from its normal of 7 to about 5 within in minutes and with the increase in the acidity of the plaque the calcium phosphate of teeth dissolves, therefore causing cavities.

HELLO,

DENTAL CAVITIES ALWAYS BEGIN FROM THE OUTSIDE OF A TOOTH AND PENETRATE INSIDE. THESE START OUT AS A VERY SMALL POINT OF DE-CALCIFICATION OF THE OUTER ENAMEL LAYER AND GROW BIGGER UNTIL THEY GET TO THE INNER DENTIN. IF NOT REMOVED THEY KEEP DAMAGING DEEPER AND DEEPER UNTIL THEY COME TO THE NERVE AND THEN THERE ARE ONLY 2 CURES.
ONE IS A ROOT CANAL AND CROWN, AND THE OTHER IS EXTRACTION OF THE TOOTH TO STOP THE PAIN.

I think a cavity usuall starts from a food particle left between the teeth or gum line. Caused by not brushing or flossing properly. If the food stays there between the teeth, they will start to rot.
