I've taught 100s of laryngectomees and vocal cord paralysis patients how to use an artificial larynx (also known as an electrolarynx). Several different models, and a couple of types exist.
A "Cooper-Rand" has a tube which delivers an artificial voice (a droning sound produced by a battery pack) and is inserted in the mouth. They can be used immediately after surgery and are very durable.
Various manufacturers produce devices which are held against the neck in order to produce the artificial "voice". Some of these devices have a pitch modulation button in order to add some variation in the otherwise monotonous voice. These types of electrolarynxes cannot be used just after surgery unless the have an attachment with a tube.
After healing from the surgery is complete, it takes a bit of experimentation to find the optimal placement on the neck to get the best voicing possible with the neck-held devices.
If you imagine that your tongue and lips work fine, but you have lost your voice, then you'll understand how all this works.

normally air passes thru the vocal cords and by positioning the tongue, soft palate,mouth, teeth and lips, we create sound or our voice. If the larynx is removed, a mechanical larynx transmits the sounds of air movement thru only the mouth and oral structures. The one I saw looked like a hand held microphone, battery operated, placed under the jaw line or upper throat. His voice was clear to understand, but monotone and mechanical sounding.
