I don't feel blame can be placed in a situation like this. The child joined the military knowing that he could be put in danger that would potential threaten his life. It would be insane to blame the child/soldier themselves, because they should be considered a hero...not negatively "blamed" for their own death. The parents are not to blame unless they forced their kid to join, but that's highly unlikely to happen. Blaming the government would only be justified if the person was killed while carrying out a mission that was considered illegal or wrong in some way.

The 19 year old made the decision the join the military he is the one who swore an oath to defend his country from all enemies foreign and domestic and also follows the Code of Conduct which in Article 1 says "I am an American, fighting in the forces which guard my country and our way of life. I am prepared to give my life in their defense."
You don't join the military to go sit on a beach and sip Pina Coladas.
It is like when a police officer gets killed. Who is to blame then? The family of the cop who let him go become a cop, the police chief, his division captain, or the person who killed him?

The individual that killed them, first and foremost.
Second to blame - the soldier that knowingly placed himself in danger (we all know it's a risk). Third in line, the parents that didn't give their child other options - lots of kids these days signing up are doing so for benefits/college money. Maybe if you had done the right thing and provided it yourself, they wouldn't have felt the need to turn to the government for support.

The blame should be placed on the agency that instigated the war. If the child is fighting defense then the attackers are at fault. When a war is fought as an offense or "pre-emptive strike" then the child's own government is to blame.

It is the 19 year old that is to blame. They join the military with the full knowledge that they may not return. The USA has a volunteer army. Its no ones faulty but his own.

If your 'child' joins the military, then he/she enters into an agreement where they acknowledge their duty to the constitution and follows the ultimate orders of the Commander-In-Chief. If that CIC decides to send your child (now soldier, sailor, airman or marine) into combat, well that's the deal from the start.
There's blame only if the military is sent into an illegal or immoral conflict where we have no business engaging. That is subjective.

This may seem to be patronizing or even degrading of the human spirit, but the first thing that comes to my mind is that everyone is to blame for everyone who is killed, even if it is in the line of duty or in defense.
My first thought is this:
Dostoyevsky wrote this about the same thing; do not ask for whom the bell tolls, it tolls for thee.
Also, that child that turns into an adult, regardless of age, and is able to make his or her own decisions is to blame for everything he or she does. Jesus Christ also says that we should turn the other cheek, and yet no one does it; so everyone is to blame.

Depends on the war...
If it were WWII, blame the Germans
if it were Korea, blame Truman or the N. Koreans
if it were Vietnam, blame Kennedy, LBJ and Nixon
if it is Iraq, blame Alqaeda and the Bush Admin but before them... blame your son because today it's an all volunteer deal.
