It's where they make a plan for the parents to get their stuff together while the kids are living somewhere else. Once the parents get their stuff together they send home one kid at a time and evaluate the effect on the family after each child gets home. It's tailored to each family's needs.

If the parents are willing, the social services agency can help in these situations by helping find jobs and a place to live. Their family reunion plan would cover the steps the parents would have to take before their children could return to them. If a single mom has a substance abuse problem and her children are taken from her, she would also be given a plan to complete before her children could be returned to her. This would likely involve rehab with random drug testing. Each individual case has its own set of criteria to be met in order to reunite the family. Social service agencies are generally run by people who usually have their hearts in the right place, but can still sometimes make mistakes when it comes to returning or removing children from their homes. It is a hard job and sometimes bad things happen.

I think you mean a family reunification plan - if so, it is when a child is taken away from their parent(s) for some reason. The CPS does a plan for that parent with so many points that the parent must accomplish before getting their child back, such as go to counseling, get a job, etc. Once these items are accomplished, the parent(s) can have their children back (sometimes with subsequent supervision visits to the home from the social worker until they feel the parent(s) are able to care for their child).
