If your child sin't old enough to write do you sign your name where the patient it required to sign or leave it blank?

If the child doesn't/can't sign their name, all they have to do is make an "X" there and you place your initials next to it.

Additional: You write their name where it says to print the name of the patient. Where it says to sign the name, you do as I've written above.

Usually all forms for medical stuff has a line for someone other than the patient to sign for cases of children - since they really can't sign for stuff like that since they are minors - and incapacitated people. Their should be a line that says someone else's signature, then a line for relation to patient.

One thing you don't want is for the doctor to put your name on a prescription because you might see the pills and think that they are yours. I think that you would write the child's name on it and then sign it.

Sign your child's name and then sign your name underneath as parent. There is usually a spot for you do to this. If you are unsure, ask the nurse at the doctor's office.

yes!! i think so. most forms will indicate that if the patient is a minor the guardian/parent should sign and where to sign. if you have any ??s ask the receptionist that's what she gets paid for my dear!!!

You sign your name and right Mother on it because the child is a minor.

I sign my name under guardian, and leave the other line blank

You would print the patient's name, and sign your name.

You are the parent/guardian, so you would sign for them since they are unable to sign and/or decide their treatment.
