stroke, brain hemorrhage, head injury, drug use, injured eye... need to know more of the circumstances of why the eyes are unequal (what happened)

It represents a unilateral lesion in the brain steam or in some of the nerves that innervate the eye. But definitely should be evaluated.

if you mean one is more dilated than the other, then it could mean that one of your eyes is not focused enough (if it is bad to see through that eye, it might require glasses)
it could be a sign of anything, sometimes injury to the head:
concussions, seizures, or even just staring at a beam of light too long with one eye.
