I would have to go with DR. and Mr. or perhaps The Sullivan Family, The Sullivans, or simply Sullivan listed over the address.

or just Dr. smith

Tradition is the man's name is first. (Yeah, I don't like it either.) But I'd do "Mr. John Doe and Dr. Jane Doe.".
