Vets do have to sell them or give you a prescription. Dog, however, can have the Heartworm pill if they are heartworm positive. It would not kill the heartworm, and it will not hurt the dog. I have done this under vet supervision in my rescue.
Dr. Malernee has an article about this on his website. He links you to the governments website to prove his points. And the heartworm pill only needs to be given every other month and only when mosquitoes are present, not year round.

All heartworm medications require a prescription. It is recommend that the dog be tested for heartworm every two years. I get mine from the vet and contrary to what tessi w says it is cheaper then a $100 a year she pays for the shot. I pay $82.58 for a years supply for my big dog, it is cheaper for the smaller two.
As chetco said you can get Revolution from your vet, it also requires a prescription, and is supposed to also be a flea control. I don't know what it costs but it is from Pfizer, yes the same company that has the price of Viagra at $10 a pill.

Answer above is correct. If he has heartworm already the heartworm tablets could kill him. There are alot of cheap heartworm tablets around but they are not as effective. I get my dogs heartworm shots from the vet for around $100 each and it last for twelve months and not only works out cheaper but also prevents any late treatment :)

Some vets, including my own, will sell you Revolution or Advantage Multi without seeing the dog. Those prevent heartworm without endangering your dog if he is already positive. They are a spot-on treatment, applied to the skin.
Just call around to the vets in your area to ask who carries either of those preventatives, and will sell them without a blood test.

If you have his vet records that shows he was tested for heartworm and is negative, as well as record that h/w preventative was bought for the dog, they might give you a few monthly doses.
They may also require he be retested.
If he has been tested recently and been on preventative, his previous vet may continue providing the preventative without retesting.

He is supposed to be tested for heartworm prior to taking the medication because if there are existing heartworms, the meds could make it worse and/or kill him.
You can get the medication online
( http://www.1800petmeds.com ) but they will contact your vet for the prescription anyway.
