How far away they will extradite depends on the geographical limits the judge put on the warrant. It may be that county only, it might be surrounding counties only, or it might be statewide, or it might be nationwide. (Only felonies will be nationwide)

So if he is stopped outside of the geographical limits he will not be arrested.

Means a warrant was issued from the "bench" i.e. court judge who was listening the case. All warrants are entered into a system. They MIGHT not come for him from a separate county but they might ask for the cooperation of the police from the county he resides in to pick him up. If he gets stopped for any reason such as a traffic violation and there is an outstanding warrant it will show up and he will be arrested then and there.

The willingness to provide proof as to why no one showed up in court is a bit unbelieveable. Find out who the judge was, call their clerk and provide notarized documents about the hospital stay, also go and turn himself in and have the documents to prove why you missed the court date on hand.

From experience:
If you have a bench warrant (bench only meaning issued by judge) if your name gets ran in any police database you will be held in that county until officers from county warrant is out of come to pick you up. Then you will wait in that county until you can see a judge usually the next day but if its late thursday or friday you will have to wait until monday (some counties have sunday court which is held inside jail)

when someone fails to appear in court the judge places a pick-up order or bench warrant on them so if they are caught they go to jail now and until they see the judge and this reflects on their case so if he does get caught then he needs a lawyer bad as a lawyer can use the time on the warrant that he does in jail as part of the sentence in my state the second domestic violance charge is a felony with a mand atory sentence and loss of rights like having a gun or voting sometimes if you know where he is have him go down to the court house in the morning and take his lawyer or a legal aid with him and turn himself in for thats the best outcome if caught some other way they may stack charges on him to make sure he does time

A bench warrant means they are going to find you and you are arrested at that point. Call and turn yoursef in, it goes easier. Get an attorney!
