Get a pair of tweezers and some ice. The ice is for numbing the area. If you can't grab it with tweezers, try and push around the object to push it out far enough to use the tweezers. You can also try to pinch the area if pushing on it don't help. It worked on my son when he stepped on a toothpick.

First get him an anti - tetanus shot in a hospital or dispensary. Then let the doctor take it out. Don't risk anything. If he stepped on an iron object, you're playing with his life and wasting time. He could get tetanus. Let an expert do it. Take him to a doctor and fast.

You need to take him to the dr. He may need a tetanus shot ~ It could be a number of things ~ none which are good ~ It needs to be removed. You dont want it to get infected.

Don't touch it as you could do lots of damage. Instead take him to the doctor or ER and they'll know how to treat it.
