A normal lymphocyte is about the size of a red blood cell and has little cytoplasm. The reactive lymphocyte in this image is seen in infectious mononucleosis; however, it is not specific for infectious mononucleosis. Reactive T lymphocytes may be seen in a variety of clinical conditions, including a wide range of viral infections. A reactive lymphocyte has an enlarged nucleus with abundant cytoplasm that is indented by the surrounding red blood cells. These are circulating T lymphocytes. The B lymphocytes remain in the lymphoid tissue and may actually be found in parenchymal organs, such as the liver, where they produce hepatitis.
EBV is not found in circulating T lymphocytes. EBV infects B lymphocytes, where it resides in a latent state for the remainder of the patient's life.
