Lesion is an all-purpose word, a non-specific catch-all term, we use to refer to both benign and malignant skin conditions - bumps, lumps, nodes, wounds, sores, rashes, boils - any visible abnormality of the tissues of the skin. Moles are not usually referred to as lesions. These are nevi. And skin lesions are not "usually open and have some kind of drainage". I are what I said.

A lesion can mean any number of things. It often refers to an open sore, but it can also be used to describe a tumor. Skin lesions are usually open and have some kind of drainage.

I believe a lesion is a open wound on the skin. Could also be a part of the skin that is affected with something; ie rash or other skin problem.
