I can't find anywhere on the state's website that has a form to fill out to change your name after you get married.
What forms do I need to fill out when I do get married?
Where do I get them?

You will need a copy of your marriage certificate and go to the bank, and those types of places or call and let them know. The most important thing to do is go to the social security office with your ss card, birth certificate, and marriage certificate. You will have to fill out a paper their they will ask you some questions and thats it they send you a new card in a couple of weeks. It isn't really that difficult and it will automatically be changed at the court house. Oh you will also need to go to the license branch with bc and mc to get a new license. Good Luck! I thought it would be a big deal too, but it wasn't, it was relatively easy!

Act 288 of 1939
711.1 Order changing name of adult, minor, or spouse and minor children.
Sec. 1.

The following is off the Social Security Admin Site:

To change your name on your Social Security card:

Complete an Application For A Social Security Card (Form SS-5);
Show us proof of your:
U.S. citizenship (if you have not previously established your citizenship with us) or immigration status;
Legal name change; and
Identity.
Take (or mail) your completed application and documents to your local Social Security office.
All documents must be either originals or copies certified by the issuing agency. We cannot accept photocopies or notarized copies of documents.

Documents for a name change: If you need to change your name on your Social Security card, you must show us a recently issued document as proof of your legal name change. Documents Social Security may accept to prove a legal name change include:

Marriage document;
Divorce decree;
Certificate of Naturalization showing a new name; or

Michigan's website is really bad. I couldn't find anything on the MDOT stating what the requirements to change your driver's license.

It should be the department of public records where you legally change your name and acquire the marriage license. Not the secretary of state. That is basically the DMV where you only change the name on your drivers license. You need to go to the Social security office to change your name of course too. Both changes you couldn't proceed with, without the sealed marriage certificate.

I don't think there is a form.... I think you just take your marriage license and other forms of ID with your new name of it to change your drivers license. Than you have to go to S.S. and change your name with them (check to see what they require). It is the world biggest pain to change your name. There isn't one for all form that I know of.

When you get married go to the Secretary of State with a copy of your marriage license. You also have to go to the department of social security to get a new card. Make sure to BRING YOUR MARRIAGE LICENSE. Good Luck, and Congrats-Married in Holland, MI

Make a visit to the city-county development. you may desire to stay with for a marriage license. Then touch the Social protection workplaces for a clean social protection card including your married call.

When I got married, we went to the JOP. He had all of the paperwork and we filled it out before the ceremony. Who is marrying you? They should be able to tell you. Also, when you apply for a license, they may be able to help you. Good luck and best wishes for a happy life!

go down to the county you got married in so if got married in det go to wayne county court house
