It depends on whether or not you have already been baptized and whether or not you have been a practicing Christian. You will need some period of instruction called RCIA. If you are baptized and a practicing Christian that period may be shorter than if you are nor already Christian (it depends on your parish.)
Once you have finished that period of instruction you will receive the Sacraments. If you are already baptized, you will start with Confession and then receive First Communion and Confirmation. If you are not baptized, you will be baptized and then receive Confirmation and First Communion.

You just have to talk to the local parish priest to begin the process. He will ask you some basic questions about why you want to convert, and will just get to know you better. He will sign you up for the RCIA (Rite of Christian Initiation of Adults) program, which is a series of classes spread out over a 9-month period. The classes cover all the teachings of the church and help you understand the sacraments. At the end of the 9 months (Usually done at Easter), the converts stand in the Mass and are blessed and confirmed members of the Church.

You have to be formally received into the Church. This usually happens during a Mass after a period of instruction. If you are interested, talk to a priest. Classes usually run fall-spring, so in the meantime, here are free home study courses that I found very helpful when I was converting to Catholicism:

My best friend's cousin was going to a Catholic church. I'm assuming she was converting, because they had her taking these classes. So I guess that's what you have to do.
