In American Etiquette, both the knife and the fork should rest on the plate between noon and two.

I live in the US, and I was raised to place the fork with the tines at 11:00 and the other end of the fork at 5:00. The knife should be just above it, blade facing inward, running paralell to the fork.

The real etiquette ( European) when you finished, you will leave the knife & fork between 9 and 3 position (Horizontal)
the knife above the fork with the kutting edge facing down.

I would say at the 2:00 position. If the meal was enjoyable then the knife's edge would face inward. If the meal was less than enjoyable the knife's sharper edge would face outward.
