First thing to check is to see if you are using the proper pillow. The firmness of the pillow should be matched to your sleeping style. For example, if you are a side sleeper, you should get a relatively firmer pillow so that your head and spine are in a straight alignment. If you are a back sleeper, you probably want a less firm pillow so that you head is not pitching forward when you sleep. Stomach sleepers probably use the least firm pillow. I'm a back and side sleeper, so I ended up with 'firm'. You can also try those contoured pillows but I have limited success with them.
Stress definitely plays a role, you might want to try more back strengthening exercises like Yoga. Yoga will also help relieve some of those tense muscles that are either directly or indirectly contributing to the pain.
You might also have a lot of lactic acid buildup in your shoulders. Again, stretching like what is found in Yoga should help, but be sure to drink water to flush out those toxins (don't replace your fluids with sugary sodas or juices...these are very acidic.) Eat more potassium also (bananas).

the fact that you mentioned pain b/w your shoulders indicates the effect of stress. stress related back pains start from b/w shoulders. The fact that you are 16 indicates the growing pain!!! remember the three Ss of the growing: stress, starvation and sleep. any teenager is so stressed out that they call it transitional psychosis. Any teenager is always hungry and they have a weird sleeping pattern. awake all night in summer and sleep till noon.

one of the best treatments for stress and pain is a hot bath, in the evening, with 2c. of dissolved epsom salts and soak for 30 minutes. It will also help you sleep better at night.
take a calcium and magnesium capsule twice daily to help with pain and stress.
