It depends on the size of the bomb, type of bomb, and location and time of exposure. Here is a general account:
Ground Zero - Death/Disintegration
Zone 1 - Death or Critical Injury; Blast force trauma, radiation poison, multiple degree burns.
Zone 2 - See Zone 1 in addition to Severe and Moderate injury.
Zone 3 - Minor to Moderate Injury as well as radiation poisoning and possibly 1st to 2nd degree burns. As well as signicficant chance of developing cancer or tumors (Thyroid is most common).
General Radiation Fallout - Radiation Poisoning, vomiting, fatigue, soars, as well as increased chance of developing cancer and tunmors (Thyroid is most common).

How close are you to the bomb?
Is it detonating?
Is it just sitting there?
Is it falling from the sky?
Is it burried underground?
Do you know how to spell physical or is that just a typo?

Well death.
But assuming you are farther away: flash burns, radiation burns, radiation poisoning, radiation damage to DNA resulting in both cancer and increased risks of mutation among offspring, blindness, deafness

Ever see anyone mutate before your eyes and die a horrible painful death as every cell is transformed by radiation? That's the mild side of it.
