I've given engraved flasks, engraved beer mugs, a bottle of good liquor, gift certificates to nudie bars (if they're 18, they get the juice bar/full nudity, 21 gets full bar/partial nudity), shot glasses, bar ware, cash for cabs (but I can't verify that's what it was used for!) and to my closer friends and family I've given my cell phone number and a promise to pick them up at whatever time, within 50 miles, no questions asked..

Free alcohol is always nice even if they have already experienced it. If you don't want to do that, a gift card, such as a Mastercard gift card would work so that he could get what he wants without you stressing out about getting him something nice and useful.

A cool sunglasses
His favorite Football/basket ball ...etc player jersey
A silver nice bracelet or necklace

Keep him out of a bar. Liquor stupidity might strike and he'll decide to have 21 shots because its his birthday.
Or if he does go to a bar keep him from drinking and driving

Tickets to a sporting event (like pro football or basketball...or college if he's a college fan).
Alcohol is nothing new for a 21-year-old. Most people drink long before they turn 21 so I wouldn't give anything like that.

I think a trip to the strip club is a good idea but go to a classy one.... you dont want anyone to get horny, drunk & beat up... just horny and drunk.
