Aside from direct loss of transmission lines, etc., the most likely cause of blackouts is failure of one generation unit in a grid so that overload occurs as power is moved in the grid to make up for it. The blackouts that cover several states in the U.S. are the result of such a failure and the hardware's reaction to the overload, cutting out plants to protect them from essentually being short circuited. It then takes time up to hours to get the plants back on line and synchronized with the network.
