Put zero on line A, then no entries until you get to line H which will also be zero. Then you'll put zero on line 5 of the actual form.
It probably won't matter a lot. If you are 14, I hope you won't be working very many hours - if you make under about $5500 for the year, you should get everything back that's withheld for federal income tax - you won't get back what's taken out for social security and medicare.
If you have anything withheld for federal income tax, you'll have to file a tax return next year to get it refunded to you.

When you first start on your new job, the payroll administrator should have you sign it right there and then have you turn it in along with all your other papers that you have to read and sign when you first start a job. I have never had to bring one home to fill out before. Also, she can assist you in filling it out if you need help!
However, since you are only 14, you should put 0 down for number of Federal allowances. I'm assuming your parents are claiming you as a dependent since you are still so young, so just put 0 on line 5 where it says "total number of allowances claiming".

What do you mean, what do you put for "A"? The lines are numbered 1-7 and then you sign the form!
Line 1: Name
Line 2: SS number
Line 3: Put "single"
And on lines 5 and 6 put 0 and then sign and date the form.

Single/1 would be typical as a dependent with one job, unless you have significant other income with no withholding. But if you later get a different job, you should likely use single/0 for that job until the new year.
