They're both the same. Wheat is a grain.

Bread tastes better when you just use the inner, starchy part of the grain. But it's more healthy to get the fiber and nutrients from the outer shell of the grain, too. It's like apples. It's healthier if you eat the apple skin, even though it's not as tasty.

So whole wheat foods incorporate the entire piece of wheat, when they make the product. "Whole grain" is just a more general term. You can use "whole grain" to describe things other than wheat, like rice or oats. "Whole grain rice" or "whole grain oats". So "whole wheat" is specifically describing a food made with wheat. But "whole grain" can be describing something made with wheat, or other grains like rice or oats.

Whole wheat, because whole grain doesn't mean that everything in there is natural and healthy.
