The worst blunder I ever made was when I worked at a Red Barn fast food restaurant at the tender young age of 17. I was tending the salad bar & accidentally lost my footing on the wet floor & sent lettuce flying all over the customers waiting in line.

One time at a buffet restaurant I got up from my chair to get more food and I bumped into someone walking with a full plate of food and about half of it ended up on the floor.

How many would you like to hear from me? :)

The worst was I had just gotten out of the hospital for MS and being blind in one eye, but I really had to keep working. I made a strawberry daiquiri for this one guy and my depth perception was off and I didn't set it on the table, I dropped it on the floor and it splashed all over him! I apologized like crazy and started crying. He said don't worry. But it was so humiliating and embarrassing.

This is awful, but once when I was working at a restaurant someone called in a togo order and I completely forgot to put in the order for one of their salads. I realized it at the last minute and punched it in. Right as they were pulling in I was hurrying to pack it up and knocked it over on the floor. I looked around and no one was in sight but one cook back in the kitchen. I packed it up, rearranged the chicken on top and sent it out anyway. I'm awful. Hey cut me some slack I was a teenager. :)

I called attention to a roach crawling on the floor of an Asian restaurant. The lady caught it with her hands in front of the whole restaurant.

Once a server dropped some flatware and when she bent over to pick it up she passed gas in front of my table. It was in an upscale dining environment and everyone there in my section stopped eating and the place became silent. I asked her if she knew where the restroom was and she pointed at it and I told her to go in there and safely remove the crappage that had to be in her underware and we got up and left.
