Trans fat is a fat product made by partially hydrogenating the fat, adding hydrogen to fat molecules. This modified fat does not digest with the normal digestive time frame as "regular" fat and attaches itself around the torso. It raises the risk of cardiac problems as well as circulatory plaque. Almost all food makers are eliminating trans fat.

it's about the trans configuration of the Hydrogen -carbon double bonds.

food that contain transfat do so so that the fat has a higher melting point and will keep longer.
The downside is that at they tend to clog the arteries.
