She is Thatcher's daughter that he had with Meredith's stepmom. In an earlier episode in the season, it was mentioned that they had a daughter that was in medical school named Lexi. I think it was the episode when Susan and Thatcher's daughter was in the hospital. It might have been in season 2, I can't remember exactly.

She used to be on that show REunion she was the shy girl that ended up dating the woman beater. The show didn't last long I think it ran like 5 episodes.

i haven't watched the season finale yet but have you heard the previous episode about meredith's step sister becoming a surgeon and going into her residency i'm thinking that it is meredith's sister.
