A song by Vasudeva Ghosh ends with the statement that

Vasudeva Ghosh says with folded hands, "He who is Gaura is He who is Krishna is He who is Jagannatha."

Lord Krishna wanted to understand the Bhakti of Bhaktas and thus in order to understand, he took on the form of Sri Chaitanya Mahaprabhu and played the role of a Bhakta trying to understand the emotions, happiness and glories of the topmost devotees, especially Srimati Radharani.

Krishna has unlimited forms, all identical. For each emotions he takes on a particular form. When he was angry towards Hirnayakasipu for torturing Bhakta Prahlada, he took on the angry form of Narasimhadeva. Similarly when Krishna wanted to understand the emotions, happiness and glories of the topmost devotees, especially Srimati Radharani, he took on the form of Sri Chaitanya Mahaprabhu and acted exactly as a devotee so that no one could detect that he himself was incarnation of lord Jagannath.

Prahlad offers prayers to Narasimhadeva in Srimad Bhagavatam 7.9.39:
In this way, my Lord, You appear in various incarnations as a human being, an animal, a great saint, a demigod, a fish or a tortoise, thus maintaining the entire creation in different planetary systems and killing the demoniac principles. According to the age, O my Lord, You protect the principles of religion. In the age of Kali, however, You do not assert Yourself as the Supreme Personality of Godhead, and therefore You are known as Triyuga, or the Lord who appears in three yugas.

As the Lord appeared just to maintain Lord Brahma from the attack of Madhu and Kaitabha, He also appeared to protect the great devotee Prahlada Maharaja. Similarly, Lord Caitanya appeared in order to protect the fallen souls of Kali-yuga. There are four yugas, or millenniums—Satya, Treta, Dvapara and Kali. In all the yugas but Kali-yuga, the Lord appears in various incarnations and asserts Himself as the Supreme Personality of Godhead, but although Lord Sri Caitanya Mahaprabhu, who appears in Kali-yuga, is the Supreme Personality of Godhead, He never asserted Himself as such. On the contrary, whenever Sri Caitanya Mahaprabhu was addressed as being as good as Krishnaa, He blocked His ears with His hands, denying His identity with Krishna, because He was playing the part of a devotee. Lord Caitanya knew that in Kali-yuga there would be many bogus incarnations pretending to be God, and therefore He avoided asserting Himself as the Supreme Personality of Godhead. Lord Caitanya Mahaprabhu is accepted as the Supreme Personality of Godhead, however, in many Vedic literatures, especially in Srimad-Bhagavatam (11.5.32):
krishna-varnam tvisakrsnam
sangopangastra-parsadam
yajnaih sankirtana-prayair
yajanti hi sumedhasah
In Kali-yuga, intelligent men worship the Supreme Personality of Godhead in the form of Sri Caitanya Mahaprabhu, who is always accompanied by His associates such as Nityananda, Advaita, Gadadhara and Srivasa. The entire Krishna consciousness movement is based on the principles of the sankirtana movement inaugurated by Sri Caitanya Mahaprabhu. Therefore one who tries to understand the Supreme Personality of Godhead through the medium of the sankirtana movement knows everything perfectly. He is sumedhas, a person with substantial intelligence.

He was an incarnation of Lord Krishna, and exhibited the mood of Radha (who is a great devotee of Krishna) But Lord Jagannath is Krishna so I guess you could say Chaitanya Mahaprabhu is an incarnation of Jagannath.

The Chaitanya Mahaprabhu is actually not an incarnation of Lord Jagannath, But he was attached to Lord so closely and he joind the lord in all relevance as a part.

The founder, Chaitanya Mahaprabhu or Lord Gouranga, was born in 1485. HE WAS REGARDED AS AN INCARNATION OF LORD KRISHNA. He took Sannyasa at the age of twenty-four. He went to Jagannath where he taught Vaishnava doctrines.

The Chaitanyas worship Lord Krishna as the Supreme Being. All castes are admissible into the sect. The devotees constantly repeat the Name of Lord Krishna.

Chaitanya’s Charitamirita by Krishna Das is a voluminous work. It contains anecdotes of Chaitanya and his principal disciples and the expositions of the doctrines of this sect. It is written in Bengali.

The Vaishnavas of this sect wear two white perpendicular streaks of sandal or Gopichandana (a kind of sacred earth) down the forehead uniting at the root of the nose and continuing to near the tip. They wear a close necklace of small Tulasi beads of three strings.

Sri Krishna and Sri Chaitanya Mahaprabhu both taught Bhakti yoga, the spiritual practice of connecting with God through devotional service to Him. Based on Their teachings, in 1966 A. C. Bhaktivedanta Swami Prabhupada, referred to as Srila Prabhupada, founded the International Society for Krishna Consciousness (ISKCON) in New York City.

Srila Prabhupada and his disciples popularized the chanting of the Hare Krishna maha-mantra in the 1960s and 1970s, and it spread to countries around the globe. Though the International Society for Krishna Consciousness is the legal name for the movement Prabhupada started, most people know it as the Hare Krishna movement.

Sri Krishna is the complete manifestation of the Divine- Absolute in all respects. Sri Caitanya is that same fully Complete Absolute not an expansion or fragment. What is the difference? and what is the real reason He appeared if a plenary portion of the Truth could re-establish religion?

Lord Krishna is a theoretical speaker (since He is Visaya or object of devotion). Lord Caitanya is that same Krishna with the mood of Srimati Radharani, His greatest devotee and hence He is a practical teacher (Asraya or abode of devotion). He freely distributes what has never been given: spontaneous devotion or raga (vraja) bhakti full of prema or pure love. Usually vidhi or religion according to rules and regualtions are given by incarnations. But Mahaprabhu gives this pure love of God or Krishna Prema indiscriminately. This has infact never been given before- it was briefly mentioned in the vedas in Sri Krishna's pastimes but by Mahaprabhu's life and teachings it has now become manifest fully in:

Chaitanya mahaprabhu was incarnation of shri Radha krishna,since krishna is lord jagannath itself so ,yes he was incarnation of lord jagannath.

As far as it is understood sri Krishna Chaitanya Mahaprabhu a devout Vaishnavite was responsible for giving utmost importance to sri.Krishna Bhakthi-infact he is considered as the father of Bhakthi cult (Yogam) and first to introduce dancing in ectacy with bhakthi songs on sri.krishna
