You don't start the conversation.....
Once your other job offer is FINAL, you will inform your boss two weeks in advance that you will be resigning. Follow that up with a written notice.
Do not do this, until the other job offer is FINAL.
Your current boss has an option of letting you go right away, so don't be surprised if he shows you the door immediately.
The letter should be simple and to the point. Remember, this is NOT the time to air your gripes. You'll be surprised how people come back into your life at later date. Simply say, the letter is to resign and your final date is . Thank for the time during the employment. That is it.
Be sure to be courteous during your remaining days. You are not obligated to tell anyone why you are quitting or where you are going. Those things are usually best left UNSAID.

Wait for the two week prior. Write it out, and then go up to him and say, I have been offered another job and would like to give you my two week notice. Give him the written explanation. If he is a jerk he might yell a bit, or chastize you..Do it at the very end of the work day. The letter should inclue your final day of work in writting, as well as a thank you for the job and opportunity. I'm sorry I have to leave but this is just such a great chance for me... blah blah blah. Wait for the last day to tell him off.You don't want those 2 weeks to be hell.

Tell him you are quitting and turn in your notice of resignation. If you have an employment contract, then follow the procedures outlined in that contract. If there is no contract, then two weeks is the standard time to give your boss to replace you. Don't be surprised if you are told to get out immediately.

You don't have to have a conversation if it makes you uncomfortable. You can send an email stating your resignation and your last day of employment (give 2 weeks notice). You don't have to give a reason or state where you'll be working next. It's none of their business.

Yes definitly write a letter and keep a copy. Tell him that you need a private moment with him and that you need to put in your notice because you have found another job. Keep it short and to the point. DOn't need to say why or be negative. Just that you are ready to move on and do something else somewhere else.

I agree with tk.
Most places want notice in writing, but make sure you are very polite - I know right now quitting in a FUN way might seem more desirable, but these things have a way of coming back to haunt you...

Only if you found another job and don't need his reference:
Resign via e-mail - I did when I worked for an insane control-freak woman. It was great. Two weeks notice tops, sooner if possible.
